Most agents are approved on a test set and then left alone. The policies, tools, workflows and conditions around them keep changing, and behavior drifts away from the system that was tested. Nobody is alerted, because the agent is still working.
That is why governance has to sit in the path of every action rather than in a document. Policy that only exists at approval time cannot see the run that happens six months later.
When each agent is built separately, guardrails, approvals and access rules are rebuilt for every use case, and they diverge. AIOS runs every process through one core under one policy model, so a rule written once applies to the phone call, the text message and the scheduled job alike.
